By completing this form, you understand that your dog will be spayed/neutered, vaccinated and microchipped by a veterinarian at Castlerock Vet and that MAPS has agreed to pay for all costs associated with the appointment.

As part of this agreement, you understand that if you do not show up for your scheduled appointment at Castlerock Vet, this service will NOT be offered again.
